Co-production

We believe people receiving care, their families and carers should have a voice in the care they receive and in how our services develop.

Co-production means working together to make decisions, solve problems and improve care. It means recognising that patients, service users, families, carers and staff all bring different knowledge and experience.

Your voice in your care

You know your own experiences, strengths, needs and goals better than anyone else.

We want you to be involved in decisions about your care wherever possible. This includes understanding what matters to you, setting goals and considering what you need to make progress.

Your care team will work with you to understand your priorities and how you want to move forward.

Working with families and carers

Families and carers can have valuable knowledge about the person they support.

Where appropriate and with the patient's consent, we involve families and carers in care planning, recovery and discharge planning.

This might include sharing information, taking part in meetings, helping the team understand what is important to the patient and supporting plans for life after St Andrew's Healthcare.

Preparing for life beyond hospital

Co-production does not stop at treatment.

As people make progress, we work with them and the people around them to prepare for their next steps.

  • Depending on the individual, this may include:
  • Developing everyday living skills
  • Building confidence and independence
  • Strengthening relationships
  • Developing community connections
  • Accessing education, volunteering or work
  • Planning accommodation and support
  • Working with community services
  • Preparing for discharge

Our social work and occupational therapy teams play an important role in bringing together the patient, family, care team and relevant services to support this transition.

Working with communities

Recovery often involves more than the support available within hospital.

We work with community teams, families, carers and other organisations to help people build connections and develop the skills they need for life in the community.

This can include supporting people to use community facilities, take part in activities, develop relationships and gradually increase their independence.

Sharing your experience

There are several ways patients, families and carers can help us improve our services.

These include:

  • Patient feedback
  • Community ward meetings
  • Patient forums
  • Carer forums
  • Peer support
  • Co-production projects
  • Service improvement projects

Patients can also share their views through a feedback system called My Voice. This can be accessed here [link to My Voice], ward tablets or completing paper copies.

Our Co-production Network

Our Co-production Network brings together patients, service users, carers, staff and volunteers to drive strategy, inspire hope, share ideas, discuss projects and overcome any barriers.
